NHS Early Rehabilitation Gym Strength Class for Musculoskeletal Conditions

Strength training is widely recognised as the foundation for improved physical function and overall well-being. Herefordshire and Worcestershire Health and Care NHS Trust has introduced a groundbreaking approach to early rehabilitation by incorporating strength classes at local gyms. These classes cater to people of different ages, with musculoskeletal conditions such as lower back pain, hip and knee osteoarthritis, and anterior knee pain. By leveraging the power of supervised exercise, the NHS aims to promote long-term self-management and positive outcomes for patients.

The Power of Strength

Scientific studies indicate that utilising lower repetitions and higher loads, optimises strength development. Remarkably, even postmenopausal women over the age of 58 with low bone mass have shown significant improvement through strength training using gym equipment. Understanding the significance of strength in improving physical function, the Herefordshire and Worcestershire Health and Care NHS Trust has taken a proactive approach to incorporate strength and conditioning classes into the rehabilitation process.

Classes and Patient Engagement

The strength and conditioning classes are conducted at selected gyms in Evesham and Droitwich on specific days of the week. Since they began, these classes have seen a remarkable response, with 420 patients referred by physiotherapists to the Rivers gyms. Every patient who joins is encouraged to attend twice a week, on a long-term basis. This sustained commitment allows patients to experience the long-term benefits of the programme, and develop self-management skills for their musculoskeletal conditions.

The Class Structure

The strength classes focus on targeting the lower quadrant muscle groups, which are commonly addressed in one-on-one physiotherapy sessions. A set of six essential exercises is used during the classes, with patients instructed to perform three sets of five repetitions. As patients progress, they gradually increase the weight, to ensure ongoing improvement. A patient progress sheet is provided to document machine weight progression, enabling personalised tracking and analysis.

Positive Outcomes and Patient Testimonials

The strength classes have demonstrated significant improvement in musculoskeletal conditions. Most patients have reported positive outcomes and expressed a desire to continue managing their conditions through gym exercises in the long term. This approach empowers patients to take charge of their health, potentially reducing waiting lists and the need for initial one-on-one physiotherapy appointments. Here are a few testimonials from patients who have benefited from the strength classes:

  • “Hats off to the team. I would never have got into the gym. The gentle nudge and encouragement have made a huge difference. Thank you.”

  • “Best thing I have ever done.”

  • “I’m so glad I came to the class. I was having terrible back problems, and now I have none at all and I’m back to using the gym regularly. Great group.”

  • “Coming to the gym has been fantastic. It has helped me drive and walk. It has helped me feel better in myself. Meeting new people! I will continue to come forever. I have also made a new friendship – one of the best parts about the class!”

The Way Forward

The success of these strength classes opens up possibilities for implementing similar programmes on a national scale. Such initiatives would provide favourable outcomes, empower patients for long-term self-management, and potentially reduce waiting lists by bypassing the need for initial one-on-one physiotherapy appointments. The Herefordshire and Worcestershire Health and Care NHS Trust has witnessed numerous success stories, including an athlete returning to competitive sports and patients experiencing significant improvement of long-standing conditions.

Strength training plays a vital role in early rehabilitation for various musculoskeletal conditions. The Herefordshire and Worcestershire Health and Care NHS Trust’s innovative approach to incorporating strength classes at local gyms has yielded promising results, with patients benefiting from improved physical function and long-term self-management. By expanding similar programmes nationwide, we can potentially revolutionise the way we address musculoskeletal conditions, empower patients, and improve overall healthcare outcomes.
